Helping Eligible Individuals and Families Access Nutrition Support

Prime Top Care is committed to helping eligible individuals and families access nutrition support and food-related services that promote health, wellness, and stability.

Access to healthy food is an important part of managing overall health and supporting positive outcomes for individuals facing certain health and social challenges.

Our team provides guidance and support to help eligible participants access available food support services and resources.

Who May Benefit?

Food support services may be available for eligible individuals who are experiencing food insecurity and have qualifying health or social needs, including:

  • Individuals with chronic health conditions

  • Pregnant individuals

  • Postpartum individuals and families

  • Individuals with identified health-related social needs

  • Have New York Medicaid Managed Care.

Eligibility requirements apply and services are based on program

guidelines.
